A quick trawl through the internet using Google will provide you with plenty of links relating to Model Releases. They are all broadly similar, but I would suggest that you look at what sort of advice the various stock photography agencies give in this regard. After all, they have a vested interest in this. Alamy, one of the largest online stock photography sites has some very useful information in this regard and provides sample model and property releases in PDF format - take a look at http://www.alamy.com/stock-photography-rel-guide.asp

Can one assume the important bit, is getting the release signed... and as to before or after this will no doubt vary for a number of reasons;

The model may want to review shoot images particuarly on a tfp based shoot before signing off.

On a commercial shoot the photographer may want to protect against a waste of resources and hence will ask for the resease pre shoot.

Personally I believe you have to trust the person you are working with to do the right thing and that goes a lot farther than formal procedures. I have on ocasion forgotten to get a release signed and have never had a problem getting one on request.

I am sure there will always be exceptions or extreme cases, where someone will wind up saying if only "....."  stuff happens but hopefully only in a small minority of cases.

I Have to agree with Carrie. The model puts her trust in the photographer during the shoot and won't always be aware of what is visible from the particular angle of the shot, surely it benefits both the model and photographer if the shots are reviewed first and then the release agreed upon as this protects both reputations and prevents any embarrasing misunderstandings.
